What is Support Coordination?
Support coordination is guidance and planning. We help you understand your DDD options, create a support plan, and connect to providers—like self-direction, day programs, employment supports, individual supports, and community activities.
A good coordinator works with you, not for you.
You should always have choice and control. Our job is to make the process clearer, keep things moving, and help you build supports that fit your life.

Am I eligible?
To work with us, you must be eligible for New Jersey DDD services and need support coordination.
If you’re not sure, that’s okay—many families aren’t. We can help you understand what DDD generally looks for and what your next step should be.
Good to know: DDD rules can feel complicated. Our role is to help you navigate the process and understand your options.

Let’s Build a Plan That Works for You
How to get started:
1. Schedule a call so we can learn your situation and answer questions.
2. We meet and gather details about needs, goals, and what’s working (and what isn’t).
3. Build your plan (ISP/PCPT) based on your priorities and DDD requirements.
4. We identify providers and supports that match your goals.
5. We submit for approval and adjust as needed.
Typical timeline: We aim for plan approval within about 30 days, depending on your availability and how quickly decisions can be made.
Common reasons plans take longer: scheduling challenges, waiting on information, or needing time to choose services.
